    About the My Supporter List Toolkit

    Planning ahead and keeping track of the people who support you can make things easier when decisions need to be made about your care, services, or daily life. The My Supporter List is a tool to help you record the names of the people who support you in different ways.

    This might include family, friends, carers, or people you’ve formally authorised through government programs or legal arrangements. Having a list in one place can help you stay organised, communicate clearly with providers, and make sure those important to you are recognised in your care and decision-making.

    For each type of supporter, we’ve included:

    • a short explanation of their role and what they can (and can’t) do,
    • links to further information, and
    • a space for you to add the names of your supporters, so you can create a personalised list to print and keep.

    Important things to know

    Not a legal document

    The My Supporter List is for your own planning. Any print-out you create from this tool will not be a legal document and will not give anyone new powers or authority.

    Does not replace official processes

    Listing someone here does not register them with any government agency, service, or company. If you want someone to act on your behalf, you must still go through the official process for that organisation.

    Seek advice if you’re unsure

    If you are uncertain about who can act on your behalf, or how to appoint or remove supporters or substitute decision-makers, you may want to:

    • contact a lawyer or seniors legal service,
    • speak with the relevant government agency, or
    • call the free, independent Aged Care Advocacy Line on 1800 700 600 for guidance.
